What does good documentation actually look like in HTM – and why does it matter?

CMIA Greater San Francisco Chapter presents “If It’s Not Documented, It Didn’t Happen: HTM Edition” with Nader Hammoud, Ramana Sastry, and Arnold Charaschirakul — a real-world look at documentation, from everyday work orders to AEM and regulatory expectations.
We’ll cover what makes a strong service record, common documentation gaps, and what to do when a surveyor says: “Show me the record.” Expect real scenarios from the field and an open Q&A, so bring the challenges you’re facing at your own facility.
CMIA members, prospective members, and anyone curious are welcome. Come connect and walk away with practical takeaways for your team.

Getting There
For those driving: Free parking is available in the lot outside the venue after 2pm. It’s first come, first served, so plan to arrive a little early if you’d like to grab a spot.
For those taking public transportation:
- BART: Civic Center/UN Plaza station is the closest stop, about a 10–15 minute walk (or a short rideshare/bus hop) from the venue.
- Muni Bus: Several lines run along nearby Bryant, Harrison, and Folsom Streets, including the 12 and 27.
- Caltrain: The 4th & King station is roughly a 10-minute walk away, a good option for those commuting from the Peninsula or South Bay.
